I didn't mean the way he was holding his head. I meant the way (it looked like) the head had been put on(ie. attached to) his body.

I thought that was what you meant by: the head was added in a different way.

It is a different head голова другая. i agree that it wouldn't make sense to interpret наставлена as the way he was holding his head - it makes it sound as though he's got two of them.

Don't worry. I've read this all again and I've got it now. откуда-то со стороны refers to where his (different kind of) head comes from. It means from some unknown place, presumably a place where they have strange heads.
