Here is the first bit (Someone correct me if it's wrong!!) so you can see the basic structure. Then you can give the rest a try and they will correct your efforts.

Do you have fun? = est-ce que tu t'amuses ? or you can also say = T'amuses-tu ?

How do you have fun? = Comment t'amuses-tu ?

I have fun by watching movie = Je m'amuse en regardant un film

Did you have fun? = Est-ce que tu t'es mausé ? Also = T'es-tu amusé ?

I had fun = Je me suis amusé ( for a boy) Je me suis amusée ( for a girl)

How did you have fun? = Comment t'es-tu amusé(e) ?
