Lithuanian Passport Application Translation

Does anyone know if there is a translation into English of the Lithuanian passport application? The document (in Lithuanian) can be found at: http://www.migracija.lt/md/Blankai/PasoPvz.doc Thanks!

I haven't managed to find such translation... I think it could be that there is no such thing at all and you just have to ask someone who speaks lithuanian to help you with filling in that lithuanian one. The same thing should be with application for citizenship, although I sent the translation to your email

Labas,

I am a Canadian with Lithuanian grandparents and am therefore eligible for Lithuanian citizenship. I received my application forms yesterday to find they were all in Lithuanian, not that I was surprised. Unfortunately though, I never learned how to speak it and cannot work through the forms as they are.

I was wondering if anybody out there had the exact translation for these papers?

Please let me know as you as you can?

Hi, I've managed to edit the document and clear out the completed fields.
Just need it translated now....

Hi,
I've translated the passport application form for you and I'm attaching it in jpg image format

Several places are left untranslated, because they are filled by officials, so there is no need for you to know what they mean

Hi There

I am from South Africa and I am eligible to apply for a Lithuanian Passport.
All I am looking for is the actual form to apply for my passport.
I have looked at the form posted previously on this thread and I am not 100% sure if thats the actual form?
If anyone has done the application for the passport, please can you help me.
I have all my papers and birth certificates of grandparents in Lithuania, all I am after is the form.
