boobs is women's bosoms, the picture says "place boobs here" in the man's chest, because man wants to feel women (body touches and sex implying),

the hat says "free mammogram", what is mammogram? it is graph of mimi (bosoms), so it tells that man wants to look at beauty, this is visual.

The subject of this thread is "double wishes of man", what is double? it is the man's two demands towards woman: visual and touch or feeling.
