Si certifica che il Sig. xxx ha passato l'esame di Assistente Dottore rilasciato dalla Facoltà di Medicina xxx e che è idoneo per le mansioni di assistente .

il sigillo della Facoltà è stato apposto sulla fotografia del detentore e
sulla firma del chairman (if you mean the Headmaster of the Faculty , you have to say Rettore). Senza il suddetto , il certificato è da considerarsi forged (sorry i don't know the meaning of this word).

"forged" in italian is "falsificato", "contraffatto"
