Need help translating!!!! Pleaase someone help me!!

Hey there, I was wondering if any would be able to translate this for me as i have been looking for an English version for two weeks with no luck and am beginning to wonder if I will ever find one. If someone could help me by going into paint or something and replacing the Japanese words with the English version I would be forever in their debt. This is the image below \/

I am not sure if you still need it, but here you go. If any of the translations seem wrong, they probably are wrong. Some of the kanji in the image were blurry and hard to read; so don`t go trusting me and blowing up your car or something.
