This is to certify that Mr. xxxx duly passed the examination for Dispensers (assistant of Doctor) certificate held by the xxx Medical Faculty and that he is fit person to perform the duties of a Dispensers.

The seal of faculty has been affixed on the photograph of the holder and signature of the chairman. Without this, the certificate shall be considered as forged.
